Amaya Make Book sample or example?

This sounds like a really basic question, but I'm an experienced
webmaster, and I can't get "Make Book" on AmayaWX 9.1 to work. No
messages, nothing seems to happen.

Is there a sample or example somewhere that I can copy?

For reference, I'm copy-and-pasting that I created from scratch.

I received some commentary about XHTML over there, but nothing on
Amaya. (I'm not sure that the posting is working on the amaya-docs
list over at W3). All that is available on the web about Amaya Make
Book seems to be unsubstantiated, i.e. it's so intuitively obvious that
I'm just plain dense, or no one has actually used the Make Book
function in 2 years.
