Automated Questionnaire

I want to code an application in asp.net which is a questionairre having the
foll features
a) The questions are such that they spawn another question in which case all
the questions can be loaded altogether n then thrown onto the screen but need
to avoid overheads in fetching the data in doing so
b) There can be radio buttons or textboxes within a question (dynamically
based on the question)
c) The questions will be so called "intelligent questions".

Has anybody come across such kind of a questionairre as the fetch n display
is simple but having the above mentioned functionalities would be rare.?

Need help and suggestions on the same.
