Cutomer-Form with more 1:n relations

hello,
i have a general question - whats the best way to implement this online form
The customer have to fill this online form in a private area.
Example of datatypes which must be coleccted
A) general data - no problem (Name,CustNr,Tel,Fax .......)
B) specific contact persons (Name,Tel,Email ...........) !!!!!
In this case a small company has only one contact - but a large could habe
still 15-20 contact persons for different areas:

What is the best way without implementing a static page with the max nr of
contacts.

Such 1:n informations are more on this online form.

Any example, any component - any idea.

in the form there are some "1:n" information to be filled

in the form there ares some blocks:
DepartmentName1,Person1,Email1,Tel1 ......Fax1, ....

where coud be only one line for some customer - but coud be in the extrame
case 15-20 lines for other... customer
DepartmentName1,Person1,Email1,Tel1 ......Fax1, ....
DepartmentName2,Person2,Email2,Tel2 ......Fax2, ....
.........
DepartmentName20,Person20,Email20,Tel20 ......Fax20, ....
