I've put together a PDF document that shows how it is possible to use
JavaScript to deliver an interactive listing in a typeset paper by
manipulating widget annotations, see:

I would be interested to hear whether or not people find its
reponse time acceptable. This particular listing has 2040 widgets
on a page, which is way more than Adobe had in mind when
they provided support for /Annots, hence this request.
