Hello Adil,
this is my suggestion:

1-
drdMedicine.Items.Add(New ListItem("All", "0"))
drdMedicine.Items.Remove(New ListItem("All", "0"))

2-
drdMedicine.Items.Add("All")
drdMedicine.Items.Remove(New ListItem("All")
