user input forms..please HELP!!!!!!!!!!!!!! :)

can anyone tell me how to popluate a dataset from a user input form?
or create an XML document from the input form that can be display in a tabel
for user verification.
once they verify the info, it can then be sent to the payment gateway.

any ideas/help or pointers will be appreciated.
