Want Div to UnHide in center of screen

I have seen examples of hiding and showing a div

What I want to do is have a table with six columns.

The sixth column is a check box to Unhide a div that is a form. I want
it to show on top of the table to enter in comments. Then click submit
in the div form and hide the div.

Pop up window is not what I want. I dont want the window like border.
Almost like a floating frame in the middle of the table.

I need the attributes for the div or css for div or javascript appear
like a window over the table.
