Pickaxe index question - LaTeX idea needed

A while back, there was a discussion of somehow adding a bit of magic
to the PickAxe index such that is a first level entry continued across
a page break, the first-level heading would somehow repeat at the top
of the second page. So, if the class list spread across two pages,
you'd get something like:

Classes
A
B
C
------------- new page
Classes (cont...)
D
E

Now that I need to do this, I can't find the discussion anywhere. Does
this ring a bell with anyone? Can this be done in LaTeX?

You might want to take a look at the fancyhdr package. Fancyhdr enables
you to customize the headers of each page. You could try to get the
current section (or chapter) title displayed in the header of each page.
