Release form like IOS app to a group of people

Every year during a day of November, in the hospital I work for, we do free service for diabetic patients. We used to fill the patients' information in papers. This year, I created a simple app that will allow everyone to see the data at the same time and store it in a database. But the thing is that the app don't match apples rules (if I want to use an external testing using TestFlight) and I can't publish it with the business manager. what can I do?
