Why won't my Javascript function fire?

I've got a Javascript function on the form above that is supposed to
check for blank values and show an alert box if any exist. It used to
work, but now unexplicably doesn't even trigger. Can anybody help
point out what I'm doing wrong?

Too much to wade through. Post a reduced test case. The process of
creating a reduced test case will likely ferret out the problem and
then you won't have to post anything.

For a start you've got two check_form() functions.
