Openwave - one field at a time

I have created form with three fields on it, and it shows up fine on
the simulator but on the actual phones, it only shows one field on the
screen with OK button. Once user enters the value and presses OK then
they see the second field and so on.

The form has no code-behind, just three fields.

I am testing on sr58 nextel phones. I am told that these are new
phones.

Please let me know what is going on.
