The code below does not compile. How can I most easily enable the
feature I want, i.e. a template function with two template parameters,
one given by argument, one to be specified by the user?

template <typename A, typename B>
void
dummy(A &a)
{
B b;
++b;
}

int
main()
{
double c;
dummy<int>(c);
return 0;
}

That requests A to be int and B to be double.
It would seem to me then that you would want:

template <typename B, typename A> // ...rest the same
