The first two are just a stupid use of frames... the 3rd is an unnecessary
use where the designer just isn't experienced or knowledgeable enough to
know the easier/best way to accomplish what they want (make 1 change and
have it appear on all pages within a site)

unnecessarily, stupidly or incorrectly.... But as well, there are the issues
such as being unable to bookmark a frame or having the page's content
indexed in a search engine without the frame (if its indexed at all)

Before CSS came along, frames and tables were the only way to do things.
Now with a ton of options, including flash, frames are beginning to die
off.
