Nothing wrong with your script. You simply missed the closing </form>
tag for the first form (you have two on this page). Put it in the
place of your choice and be happy ;-)

The error was in the main function. Due to changes in overall logic the
subfunction that wrote the </form> tag was never executed. Now (nearly)
everything works perfect including the onsubmit.
