[OT] Tool for renaming C identifiers

(for the c.l.c. readers: Yes, I am aware this question is not about the
C language - flame me)

Does anybody know about a tool (preferrably *nix) that is able to
automatically rename C identifiers (function names, struct members, etc)
in a given list of source files ? Global search-and-replace is tedious
because this often renemes too much, breaking the code on the fly.
